Once an appointment has been set we ask that you complete the new client paperwork packet
*Please complete the new client paperwork for each individual attending therapy and e-mail (info@chariscounselingcenter.com) or fax to 407.894.5620 the completed documentation to our office before your first scheduled visit.

We have provided the PDFs in electronic format so they can also be completed using Preview (Apple) or Adobe Reader (Windows). Please Note: Some pop-up blockers may need to be disabled in order to allow the Save document window to open.
Please download these forms, fill it out in advance of your first appointment, and bring it in with you. If not, we ask that you come in 15-minutes prior to your appointment and we will provide you with a copy to fill out in our office.
Please download and complete this form when the client is under the age of 18.
Use this form to help us keep our records updated with your current contact and billing information. Completed forms may be mailed, faxed, or dropped off in person.
Due to confidentiality contraints, we cannot communicate with anyone about client information (even to acknowledge we are seeing a client) without the consent of the client (or parent/guardian of a client). Use this form to provide consent for your counselor to provide any information to a third party.
